Recovering from an illness or injury doesn’t happen overnight. If you’re not quite ready to return home after your hospital stay, our caring team at the Transitional Care Unit at AdventHealth DeLand is here to help you heal.

Located within the AdventHealth DeLand hospital, we are the only Transitional Care Unit in Volusia County within a hospital to offer physical, occupational and speech therapies seven days per week. We are the bridge that connects you from your successful treatment in the hospital to the familiar atmosphere of your home.

After hospitalization, we want to make your recovery as smooth and supported as possible. That’s why we’ve created a Transitional Care Unit right here in our own hospital, providing skilled nursing and rehab services seven days a week.

  • Question: Why is transitional care located in the hospital?

    Answer:
    Our location on the third floor of AdventHealth DeLand makes it easy to deliver the care that you or a loved one needs to renew your strength. Having our unit within the hospital affords you access to physicians, specialists and consultants, so you can benefit from having all the care you need in one place.
  • Question: How long can I stay?

    Answer:

    The average length of stay is two weeks. Our team will work closely with you to make an individual plan that best fits your needs and helps you feel healthier as soon as possible. Our goal is to help you achieve a healthy, speedy and successful recovery through whole-person care.

  • Question: Do I have to be an AdventHealth patient?

    Answer:

    You do not have to be a current patient in an AdventHealth hospital or facility to benefit from the care available in our Transitional Care Unit. Call us at Call386-943-3100 to learn more.

  • Question: Do you accept Medicare or Medicaid?

    Answer:

    We accept Medicare. We do not accept Medicaid as a primary insurance.

Feel Like Yourself Again With Therapy Tailored to You

We’ve designed our Transitional Care Unit to provide 24-hour care. Your team will answer questions, assess new symptoms and quickly notify doctors about any changes in your health status, regardless of the time of day. We’re here around the clock, so you and your loved ones can have peace of mind.

We offer physical, occupational and speech therapy to ease you back into daily life, with a broad range of other individualized therapeutic and rehabilitation services, including:
