In-Home Care for Whole Health and Healing
- Qualifying for Home Health Care
-
We accept most insurance plans, including Medicare.
Our home care services are available to people who:
- Are under the care of a physician
- Live in Volusia or Flagler counties
- Are homebound and require the skills of a registered nurse, physical therapist or speech-language pathologist
To qualify for benefits under Medicare, you must be considered homebound, which Medicare defines as:
- Requiring considerable and taxing effort to leave home
- Absences from the home are infrequent and of short duration
- A treating physician has advised it’s unsafe to leave home due to a medical or psychiatric condition
Someone who qualifies as homebound may:
- Use a walker, have difficulty going up and down stairs to leave home and need help getting in and out of a car
- Walk without direct assistance for only short distances due to shortness of breath or other physical impairment
- Have limited mobility caused by extreme weakness or severe pain
- Be confused and unable to leave home unassisted
- Be depressed, withdrawn or refuse to leave the house
Examples of allowable absences from the home include:
- Church attendance
- Beauty salon services
- Other nonmedical reasons if absences are infrequent, of short duration and require a considerable and taxing effort
- Visit to the hospital, doctor’s office or a health care facility
- Rehab in Your Own Home
-
After returning home from the hospital or outpatient surgery, you may still need support. Our AdventHealth Home Care services include rehabilitation as you rebuild your strength and re-establish your health and daily routines at home.
Whether you’re recovering from a total knee replacement or adjusting to life after a stroke, you don’t have to navigate these new challenges alone. Our occupational and physical therapists will come to you, helping you meet every milestone on the path to a full recovery at home.
