Home Care for Whole Health and Healing
- Understanding Home Care
-
Home care treats an illness or injury that has made it difficult for you to leave home. It helps you recover, regain your independence and become as self-sufficient as possible in the comfort of your home.
If you have temporary to chronic health issues, home care provides qualified medical clinicians who work with you on your path to better health. Our clinical team works with you and your doctor to evaluate your health care needs and develop your plan of care.
- Accessing Home Care Services
-
Contact our home care team via phone or by submitting our contact form. We’ll reach out to discuss what you’re looking for and how we can best serve you. Our clinical team works with you and your doctor to evaluate your health care needs and develop your care plan.
To qualify for home care, you must be:
- Under the care of a referring and following physician
- Homebound, as defined by Medicare
- Residing and receiving care within our licensed coverage area
- Rehab in Your Own Home
