Florida Hospital Flagler Announces New Nurse Navigator

PALM COAST, Fla. August 12, 2016 Florida Hospital Flagler has named Katie Fazio as the nurse navigator for cancer patients receiving treatment at the Palm Coast hospital.

In her new role, Fazio will be the single point of contact for patients and their families and serve as a liaison between patients and their medical team. Additionally, she will educate, support and assist patients throughout their individual journeys.

Fazio has been with Florida Hospital Flagler for three years and previously served as a radiation oncology clinic nurse. In this role, she was responsible for assessing for treatment reactions, scheduling scans and providing education on radiation therapy for all new patients, those requiring follow-up care and those currently undergoing treatment.

A St. Augustine native, Fazio has been a registered nurse for eight years. She graduated with an Associate of Science in Nursing from St. Johns River Community College.
