Learning experience: Ambulatory Care

Dr. Raquel Caporella serves as one of the PGY2 preceptors at AdventHealth East. She completed an ambulatory care focused PGY1 residency at AdventHealth Celebration in 2016 and has experience in anticoagulation management and transitions of care. She previously served as a primary preceptor for residents and students at AdventHealth Altamonte. She was recognized as the AdventHealth Altamonte Springs PGY1 Residency Preceptor of the Year in 2019. She served as lead pharmacist at the transitional care clinic at AdventHealth Altamonte Springs where she had the opportunity to work with an interdisciplinary team of nurse practitioners, respiratory therapists and medical assistants to provide medication therapy management services to the underserved population and patients who are newly discharged from the hospital. Additionally, she has experience working as a clinical staff pharmacist in the inpatient setting performing discharge medication reviews. Her previous research experiences involved quality improvement projects focusing on improving patient outcomes for diabetic patients by performing pharmacy driven follow up phone calls. She is excited to join the East Orlando team and looking forward to continue to promote pharmacy services. Her areas of interest are diabetes, COPD/asthma and transitions of care.
