A Pilot Program Steers Patients to Whole-Person Care Right Where They Live

Staying on top of her health and wellness is important to Pauline Corn. At 88, she is the President of the Carolina village Resident Council and is very active with her friends and neighbors at the retirement community in Hendersonville. So, she doesn’t have a lot of time to spend trying to get to and from doctors’ appointments.

Now she doesn’t have to. “I have issues with mobility so being able to avoid having to go out in the cold or extreme conditions to drive to a doctor’s appointment is a concern for me. Now all I have to do is walk down the hallway to see Sharon.”

She is talking about Sharon Kast, FNP, who provides health care at AdventHealth Medical Group Primary Care at Carolina Village. In February of this year, AdventHealth Medical Group opened the doors to its on-site clinic and Pauline was one of the first people to walk through them.

“I saw Sharon the first week the clinic opened at the Village. I was impressed by her deep concern for the patients. Everybody is excited about the fact that we have the clinic. It is so convenient for them to make an appointment and then be able to walk to the AdventHealth clinic office for treatment for whatever problem they may have and then go to our in-house pharmacy and go home and recuperate.”

Family Nurse Practitioner, Sharon Kast, is just as excited to be caring for the men and women living at Carolina Village. “I love the people here,” says Kast. “I have felt like the older populations are underserved. They often tell me they don’t feel like their health care professionals listened to them. This is the age group we learn so much from. Some of my patients here have 90 years of health history for me to discover and use to help determine their best path to feel whole. It is absolutely wonderful.”

This collaboration means patients no longer have the burden of trying to arrange transportation to off-site doctor office visits for simple illnesses, health screenings and preventive health care.

“Our goal is to keep residents independent and in their own homes as long as possible for their own comfort by providing convenient physician services. Our internal Independent Living Support team has been working successfully with the AdventHealth Medical Group team at Carolina Village, so we are able to follow residents through the continuum of care, ensuring that they receive thorough, seamless, convenient, quality care,” said Kevin Parries, Carolina Village Executive Director.

Pauline and several of her neighbors have been so pleased with convenient access to whole-person care, they are now planning to switch their primary health care to AdventHealth Medical Group and Sharon Kast, FNP. “We here at Carolina Village are truly blessed to be able to have a clinic and Sharon with us five days each week.”
