- How does choosing one of our Preferred Provider Network partners benefit you?

Having your care provided by a partner in the Preferred Provider Network means that you will receive services from partners that have proven to AdventHealth that they provide the highest quality care and are committed to continually improve.
Each provider in the network must meet and maintain standards for high quality care. Providers were chosen based on their performance, high quality ratings and willingness to build relationships.
- When will I need to decide if I want to choose a Preferred Provider?

If your physician orders post-acute care, a hospital Care Manager will work with you to determine if a Preferred Provider suits your needs.
- How do I choose a partner in the Preferred Provider Network?

A care manager will give you a list of Preferred Providers which will include information about their quality of care ratings and location. You simply let the care manager know of your choice and they will help arrange your continued care with the Preferred Provider.
- Who will provide the information to me about the Preferred Provider Network?

A hospital care manager will provide a list of options for you to choose from prior to your discharge.
- Are any of the AdventHealth Preferred Provider Network facilities owned by AdventHealth?

AdventHealth owns some of the providers and is aligned with other preferred providers as well.
- Do I have to select a provider from the PPN?

There is no obligation to choose from the Preferred Provider Network list. The network is offered to you as a guide in selecting a post-acute caregiver that has proven to AdventHealth that they provide the highest quality of care to our patients.
- Are there any fees associated with using the PPN?

There are no fees associated with using the network.
