- Bronchitis
- Chronic obstructive pulmonary disease (COPD)
- Congestive heart failure
- Coronary artery disease
- Diabetes
- Emphysema
- Hyperlipidemia
- Hypertension
- Post-coronary artery bypass grafting

Lean on our expertise to ensure you get the very best start.
- Comprehensive Medication Review
-
When you leave the hospital — no matter your condition — you’ll receive written instructions for medications you should continue taking. But that isn’t where your care ends.
Since chronic diseases often require multiple medications, frequent adjustments and ongoing monitoring, it often takes more than a pillbox to help you best manage your health. You need expert support and guidance, especially when you’re first released from the hospital and learning to live with your new diagnosis. We’re here to help.
- Thorough Diabetes Education
-
From diabetes diagnosis to transitioning into the lifelong management of your condition, our diabetes specialists are here to help you:
- Better understand your condition
- Commit to lifestyle changes that will have a positive impact on your symptoms and overall health
- Feel confident about monitoring your blood sugar
- Learn how to manage your medications
- International Normalized Ratio (INR) Testing
-
If you take blood thinners (also called anti-clotting medications or anti-coagulants), it’s especially important to measure the time it takes your blood to clot. With a simple blood test, your AdventHealth Care Clinic team can calculate your INR and gain important insights about whether your current medications are working effectively and decide if adjustments are needed.
- Resources Available to You

We have several resources available to help you get the care you need and feel whole again. These include:
