Purpose

Medication Management Services will:

• Reconcile medications and create a complete medication list for the patient
• Help assure patients receive their medications or find alternatives if patient is unable to afford them
• Assure patients understand their medications, how to use them, how to store them, and the importance of taking them
• Assess for adverse reactions to medications
• Facilitate physician follow-up
• When necessary, optimize therapy based on guidelines
• Monitor disease state progression

What to expect

Patients are scheduled for 15 minute office visits during which an instant finger-stick INR is obtained as well as an assessment of therapy. Our clinical pharmacists provide the patient with dosing instructions and may renew prescriptions for warfarin and low-molecular-weight heparins on behalf of the patient's enrolling physician as part of a collaborative practice agreement. Patients leave each visit with instant INR results, dosing regimen and next scheduled follow-up appointment. An official report with patient's INR result, assessment and dosing instructions is faxed to the patient's physician within 48 hours.
