Meet Your Care Coordinator

The Priority Health program gives you access to a team of Care Coordinators. Think of the Care Coordinator as your personal assistant for every healthcare need. They are here to provide personalized care, including but not limited to:

  • Providing priority access to a variety of specialists and primary care physicians
  • Scheduling and coordinating priority appointments
  • Providing healthcare reminders
  • Ensuring our providers align with your insurance

Priority Health allows anyone in your household to be the primary account holder. We do accept all major insurance providers, but if you are unsure, your Care Coordinator can help confirm your insurance eligibility. The Care Coordinator is available Monday – Friday, 8:00 am – 5:00 pm.
