Community it Serves

AdventHealth strives to live out our mission of Extending the Healing Ministry of Christ beyond the walls of our hospital. One such way is by working to improve the health of the communities we serve.

Every three years, we conduct Community Health Needs Assessments to determine the major needs in our area. This data is then used to develop a Community Health Plan for each hospital to improve residents’ health through education, research, financial assistance, special programs and spiritual resources. These services, referred to as community benefits, are offered without regard to profit.

Community benefits are programs or activities that provide treatment and/or promote health and healing as a response to identified community needs. They increase access to health care and improve community health. At AdventHealth, we work with a wide range of community partners to help meet the needs of our communities. Community Health Needs Assessments

Every three years, the hospital conducts a Community Health Needs Assessment (CHNA) and a three-year Community Health Plan (CHP). The goals of the assessment are to:

  • Engage public health and community stakeholders including low-income, minority and other underserved populations.
  • Assess and understand the community’s health issues and needs.
  • Understand the health behaviors, risk factors and social determinants of health.
  • Identify community resources and collaborate with community partners.
  • Use assessment findings to develop and implement a three-year strategic Community Health Plan.

Community Health Needs Assessments

The Community Health Plans, or implementation strategies, are the action plans to address the priorities identified from the Community Health Needs Assessment. The Community Health Plan outlines targeted interventions and measurable outcomes. Progress on the community health plan is continually monitored annually.
