We provide referring physicians and hospitals with 24/7 support and transfer services. Our transport and critical care teams work closely with referring doctors to ensure safe, rapid transfers based on the patient’s condition and distance from AdventHealth for Children.
Our transport clinicians — highly skilled nurses and respiratory therapists — work under the supervision of our neonatologists and pediatric critical care doctors, depending on your patient's needs.
To help us expedite your patient’s care, please be prepared with the following information when you call for a referral:
- Your name, facility and phone number
- Patient’s name, date of birth and weight
- Referring physician
- Patient’s diagnosis
Upon the transport team’s arrival, please have the following items ready to share:
- Copies of X-rays, MRI reports, CT scans or other applicable charts
- Signed COBRA and signed transfer consent form authorized by a parent or guardian
- Extra copy of the fact sheet for the transport unit
