The clinic is located at 102 Professional Drive in London, KY 40741, and is open Monday-Saturday, 8a-5 pm, and Sunday, 1p-5 pm, accepting both walk-ins and appointments available online at PrimaryCareForLife.com. The clinic's primary care providers are Dr. Jackie Maxey and Karla Sizemore, APRN. In addition to primary care and weekend services, the clinic offers Orthopedics & Spine Care, Urology, Women's Care, Behavioral Health, and General Surgery.
